Developed in early 2000, this surgery performed through key-hole route is performed world wide. The stomach is stapled along the length and the excess or remnant part of the stomach is removed. The removal of stomach is done through one of the keyhole after stretching it. This surgery is done under general anaesthesia & usual operating time is 1hr.

Mechanism of Blood Sugar Control:

  • Reduced food intake—reduced blood sugar spike after a meal.
  • Reduced Ghrelin hormone secretion, which may have certain role in blood sugar control.

Mechanisms of Weight Loss:

  • This surgery works on principle of reduction of stomach capacity, thus after surgery person feels satiated or full with comparatively small food intake.
  • The removed part of stomach is known to secrete a hormone called Ghrelin, which is supposed to make us hungry. Once removed hunger stimulation is reduced.

After surgery person feels less hungry, feels full with smaller food intake & thus has significantly reduced calorie intake. This results in calorie deficit & utilization of existing fat depots from all over the body.

Usually patients are expected to lose 65%-70% excess weight within 12-18 months after surgery. However, this weight loss may vary depending on calorie content of food consumed & physical activity levels.

Over 70% patients are expected to maintain blood sugar levels without medication after surgery. Some of the studies show diabetes resolution in over 80% patients. In patients with inadequate control, ileal transposition can be performed as 2 nd stage procedure to improve results.

Post-op Recovery

  • Immediate
    • Most of the patients can be shifted to their rooms immediately after surgery but some may be kept in recovery/intensive care by the anaesthetist due to their medical limitations.
    • Most of the patients are allowed to have liquids within 24 hours of surgery.
    • Patients are encouraged to walk by evening on the day of surgery.
  • Delayed
    • Usually patients are sent home on the 3 rd day after surgery.
    • Patients can resume normal work 7-10 days after discharge from hospital.

Diet Recommendation
Full diet charts are provided by our bariatric physician. This includes liquid diet for 2 weeks after surgery, soft diet for another 4 weeks & then normal diet. It is strongly recommended that all instructions are followed to avoid any nutritional imbalance.

Physical Activity Recommendation
During the active weight loss period, human body resists weight loss, this results in lethargy, less energy expenditure & reduction in metabolic rate. This slows down weight loss, even if patient is eating small food portions. Patients are advised to resume physical activity after surgery to ensure higher body metabolism .All recommendations are in relation to time after surgery.

Long walks : 10 days later

Aerobic activities like swimming : 20 days later

Weight training etc : 30 days later

Abdominal exercises : 3 months later

Long Term Results
Sleeve was developed as 1st Stage Procedure before duodenal switch in USA & is being occasionally used as Single Stage Surgery in some patients. Maximum 5 year follow up data is available suggesting good weight & blood sugar maintenance. However, in some patients a Second Stage Surgery may be required if indicated.
